Please note that in the case of forum bans you can have someone else post the appeal for you.

Well, in that case, I only have a few suggestions: do we really need to have them indicate why they were banned, when they might not comprehend the banner's reason for doing so, and what the IP address is? Also, "Length of Ban" is listed twice underneath the forum ban appeal...

Then can always just say "No clue" if they don't know on the ban reason, and you are right in that we can remove IP address, I'll go do that now.
I also made various corrects in that more than just admins ban people and people that aren't sparkpowder can post people's ban repeal apps for them.
Now the formats are the same, so should we just combine them?
